About Your Visit

We want to answer as many questions as possible at your visit, please prepare for this by bringing the following information. Please contact us directly at 859-781-4900 with any questions.

Things you should bring to your first visit:

  • Health Insurance Card/Information
  • A friend or family member to take notes/help with questions
  • A list of all medications you are taking
  • A list of your questions /concerns
  • A copy of your XRAYS / CT scans (not the reports - get the actual films or a CD)
  • Copies of records from doctors related to your sinuses/nose/allergies/lungs.

Office Policy

We accept most insurance policies. Co-payments are collected at the time of service.

If you are unable to keep a scheduled appointment, please call 24 hours prior to your appointment time to notify us of your cancellation so that another patient may be seen instead. Head and Neck Surgery Associates reserves the right to bill patients for missed appointments.

If you are a new patient or have not been seen at one of our offices within a 12- month period please arrive 15 minutes early to fill out /update your paperwork. Or, you may print out your paperwork and complete it before your visit. You may also complete your paperwork online.

Head and Neck Surgery Associates reserves the right to charge a fee for missed appointments, completing forms such as FMLA, or for failing to remit copayments at the time of service.

Prescription Refills By Calling The Office

Please call our office for your prescription refills requests at 859-781-4900 during normal office hours.

About Our Office Hours

In general our surgeons are in the operating room each morning, and patients are seen between the hours of 12:30 PM and 4:30PM at most offices.




Our Edgewood location
, adjacent to St. Elizabeth South Hospital, frequently has additional patient openings from 9:00 AM to 11:30 AM.

 

 



Grant county office
, patients are seen each Monday morning from 9:00 AM to 11:30 AM.

About our Allergy Departments Hours:

Our Allergy Departments perform allergy testing and give allergy injections in the afternoon when a physician is present. In general, this is from 12:30 pm to 5:00 pm. Please call 859-781-4900 with any questions.

About our Audiology and Hearing aid Hours:

Our Audiologists are available for hearing testing and evaluation for hearing aids in the morning, and work closely with our physicians when they are seeing patients, typically each afternoon. Please call 859-781-4900 to schedule an appointment directly with an audiologist, and ask our receptionist for help in determining which office is most convenient for you.
