Our Dearborn County office is open on Monday, Tuesday and Friday only from 10:00 am to 4:30 pm.
Our Grant County office is open each Monday from 9:00am to 11:30 am.

Please consider filling out forms that pertain to your office visit now. This saves you time and helps us process your paperwork faster on the day of your appointment. Please contact us directly at 859-781-4900 with any questions on these forms.

1) Print out forms now, and fill them out by hand, and bring the forms with you to your appointment. Download forms here

2) Arrive 15 minutes early to your appointment and we’ll give you the paper work to fill out before hand.

3) Or call (859) 781-4900 and we’ll mail or email you the documents.

Please print out and complete the Patient Health History and the Patient Registration Form before coming in for your first appointment.
(Note: These forms must be updated yearly. Filling them out in advance may lessen your waiting time.)

We have developed specialized questionnaires that pertain to certain ear, nose & throat disorders and diseases. If either of the following surveys pertains to your symptoms, please complete the survey before your appointment.
